Role Description
This is a full-time, on-site Care Manager role located in Ahmedabad. In this role, you will oversee case management for elderly clients, coordinate personalized care plans, and serve as the primary point of contact for family members and healthcare providers. Responsibilities include assessing client needs, arranging appropriate services, training caregivers, and ensuring access to quality elder care services. You will also work closely with social service providers to deliver responsive and efficient support for clients.
Qualifications
- Experience in Case Management and Training caregivers or support staff
- Knowledge of Elder Care practices and Social Services coordination
- Familiarity with Medicine and healthcare requirements for elderly individuals
- Excellent organizational, communication, and interpersonal skills
- Ability to adapt to on-site work and collaborate within a team
- Previous experience in a related field, such as social work or healthcare, is an advantage
- Bachelor’s degree in Social Work, Healthcare, or a related field is preferred
