About the Role
- Provides coordinated, seamless care for structural heart patients from referral through discharge and follow-up
- Works collaboratively with cardiologists, cardiovascular surgeons, nurse practitioners, and the structural heart coordinator
- Supports patient assessment, care planning, and education across the full care continuum
- Serves as a liaison between patients, families, providers, and referring facilities
- Promotes continuity of care, compliance, and positive patient outcomes
How You’ll Drive Our Mission Forward
- Assist with pre-, peri-, and post-procedure patient care activities
- Coordinate scheduling, testing, and follow-up appointments to ensure efficient care delivery
- Act as a primary point of contact for patients, families, and referring providers (including phone triage)
- Educate patients and families to ensure understanding of treatment plans and available resources
- Track patients throughout the continuum (pre-admission, inpatient stay, 30-day and 1-year follow-ups)
- Ensure compliance with Medicare requirements and structural heart registries (e.g., TAVR)
- Collaborate with multidisciplinary teams to improve care coordination and patient outcomes
- Support data collection, case reviews, and quality improvement initiatives
- Build and maintain relationships with referring physicians and external partners
What Makes You a Great Match
- Licensed Practical Nurse (LPN) with a current Louisiana license and BLS certification
- Minimum of 2 years of experience, preferably in a cardiology or clinic setting
- Strong organizational and multitasking abilities in a fast-paced environment
- Excellent communication and interpersonal skills with patients, families, and healthcare teams
- Compassionate, patient-focused approach with emotional intelligence
- Ability to think critically, solve problems, and make sound decisions
- Comfortable working independently while collaborating with a multidisciplinary team
- Knowledge of healthcare regulations, documentation, and patient care standards
- Skilled in patient education and care coordination
- Detail-oriented with strong documentation and time management skills
