<strong>Responsibilities:<br></strong><br>
</div><ul>
<li> Administer medications and treatments as prescribed by healthcare providers </li>
<li> Monitor and document patient vital signs and overall health status </li>
<li> Assist with daily living activities such as bathing, grooming, and meal preparation </li>
<li> Educate patients and families on managing health conditions and care plans </li>
<li> Coordinate with RNs, therapists, physicians, and other healthcare professionals </li>
<li> Maintain accurate and timely documentation using EHR systems </li>
<li> Provide emotional support and companionship </li>
<li> Follow all safety and health regulations in home care settings </li>

<strong>Requirements:<br></strong><br>

<li> Active and valid LPN license in Massachusetts </li>
<li> Minimum 1 year of experience in home health or related healthcare setting </li>
<li> Current BLS certification </li>
<li> Reliable transportation and willingness to travel to patient homes </li>
<li> Proficiency in electronic health records (EHR) </li>
<li> Strong communication and interpersonal skills </li>
<li> Compassion and commitment to quality patient care </li>
