Your role at BHSN:
A Care Navigator plays a vital role in supporting individuals who require assistance navigating complex social service systems and accessing appropriate resources. They work closely with clients facing various challenges, such as mental health issues and substance abuse or other barriers to independent living. The primary objective of this role is to assess individual needs, develop tailored plans, coordinate services, and advocate for clients to ensure they receive comprehensive support and achieve their goals.
Work Schedule: Monday-Friday 8am-4pm with one day being 10am-6pm

What your day might look like:
- Conduct thorough assessments to identify clients' strengths, needs, preferences, and barriers to independent living
- Gather relevant information from clients, families, caregivers, and other service providers to develop a comprehensive understanding of the client's situation
- Liaise with various community-based organizations, healthcare providers, government agencies, and other stakeholders to coordinate services and resources for clients
- Facilitate referrals to external agencies or specialists for medical, behavioral health, housing, employment, education, or other supportive services as needed
- Other job duties as assigned
Your skills and qualifications:
- Associate degree or 2-5 years in a related field required. Bachelor's degree preferred
- Valid NYS driver’s license with driving record acceptable to BHSN insurance carrier
- Reliable transportation for required travel
- Ability to work a flexible schedule to accommodate the needs of families
