As a Specialty Referral Coordinator, you’ll facilitate the flow of internal and external referrals to Avance Care Nutrition, Behavioral and Psychiatry by acting as a liaison between referring providers, specialty care providers and patients while providing high quality customer care.
This is a full-time role involving 8 hours weekday shifts with no weekends schedule.
- The position works a hybrid schedule with 3 days remote and 2 days onsite.

Selected Responsibilities:
- Ensure referrals are submitted, accepted and followed up on in a timely manner
- Provide appropriate clinical information to outside entities, as needed
- Process complete and accurate registration for patients
- Assist patients in problem solving issues related to their referrals including taking an advocate role on the patient’s behalf with the carrier to ensure approval of necessary services in a timely fashion
- Maintain detailed and accurate referral documentation and communication with referring providers within the electronic medical record
- Establish and maintain relationships with providers and outside entities
- Review details and expectations about the referral with patients as needed
- Confirm patient’s chart is up to date with consult information from other entities
- Monitor and ensure proper utilization of the High Value Specialist and Medical Neighborhood networks so patients have access to the right care at the right time
- Communicate effectively with specialist offices assuring safe and coordinated transitions of patient care
Candidates should have a High School Diploma required, or associate’s degree in healthcare administration or related field, preferred. Along with medical terminology knowledge and 1-year related experience helpful.
