How you'll make an impact in this roleHolistic Care Coordination: Lead the interdisciplinary plan of care from admission to discharge, ensuring all strategies integrate the patient’s personal values and preferences across the entire health continuum.
Specialized Clinical Support: Serve as the primary resource for assessing and treating complex psychosocial needs, including crisis intervention, substance abuse, neglect, and legal or adjustment challenges.
Systemic Advocacy: Proactively identify and remove barriers to recovery by advocating for essential resources and navigating the complexities of social and medical systems on behalf of the patient.
Strategic Communication & Documentation: Maintain a continuous feedback loop with the care transition team to reevaluate health plans and ensure all medical documentation accurately reflects the complexity of the patient's treatment and post-discharge needs.
What minimum qualifications you'll needLicensure / Certification / Registration:
- Clinical Counselor Preferred.
- Social Worker specializing in Clinical Practice credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.
Education:
- High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
What additional preferences you'll need- Previous case management or discharge planning experience
- MSW
