Position Summary
This supports the City of Glendale’s Housing Now scattered site grant, providing long-term housing and supportive services to individuals who meet the HUD definition of Chronically Homeless. Case management duties include budgeting, life skills counseling, healthcare and legal advocacy, benefits assistance (e.g., SSI/SSDI), and connections to additional resources as needed. This role uses harm reduction strategies to help participants retain their rental housing and works with individuals and families to develop personalized case plans, including housing and employment goals. The position starts as a temporary assignment with the potential for permanent employment based on performance, attendance, and program needs.
Duties and Responsibilities

Housing Counseling

Work with clients to develop individualized housing plans.

Adhere to professional boundaries and ethics.

Assist clients with housing applications and paperwork.

Survey the rental market for affordable housing and advocate with prospective landlords.

Housing Placement Support

Documentation and Recordkeeping

Maintain accurate documentation of service objectives and outcomes.

Enter case notes and data into HMIS.

Prepare reports related to case management outcomes and successes.

Home Visits and Client Follow-Up

Landlord/Tenant Relations

Attend program-related community, coalition, and committee meetings as assigned.

Conduct crisis intervention when necessary.

Case Conferencing
Qualifications and Requirements

Bachelor’s degree in Human Services or related field, or equivalent education and experience.

Experience in social service settings with knowledge of case management systems and planning techniques.

Knowledge of housing resources, tenant rights, and Continuum of Care services for the homeless.

Experience working with homeless individuals and families preferred.

Proficient in Microsoft Word and Excel.

Must possess a valid driver’s license.

Must have access to reliable transportation and meet agency auto insurance requirements.

Employees using personal vehicles must maintain active and adequate auto insurance per state regulations.
Physical Working Conditions and Office Setting

Some travel required to field locations.

Must maintain a valid driver’s license and current auto insurance.

Requires prolonged sitting, standing, bending, stooping, and stretching.

Must be able to lift up to 25 pounds.

Requires extended use of a computer, keyboard, and phone.
