POSITION SUMMARY
The Patient Care Coordinator Team Lead is responsible for guiding and supporting the PCC team to ensure patients receive seamless, coordinated care throughout their healthcare journey. In this role, the PCC Team lead will oversee the PCCs withing your region, providing them with the training, resources, and support they need to excel. You will also monitor team performance, streamline workflows, and foster clear communication across all care settings.
Essential Functions Statement(s)
Team Leadership and Support
- Oversee and monitor a team of Patient Care Coordinators (PCCs) within your region, ensuring they have the necessary tools, resources, and support to provide high-quality care coordination.
- Facilitate ongoing training and development opportunities for the PCC team, helping them stay current with best practices in care coordination, referrals, and patient communication.
- Monitor team performance, provide feedback, and promote a collaborative and efficient work environment to ensure patient care goals are met.
Patient Support and Coordination
- Assist PCCs in developing and managing personalized care plans that address the unique needs and preferences of patients, ensuring a patient-centered approach.
- Act as a secondary point of contact for escalated patient concerns, offering guidance and resolving issues that PCCs may need assistance with.
- Support the team in facilitating communication between patients, healthcare providers, and other stakeholders, ensuring smooth and timely information flow.
Care Plan Management
- Oversee the monitoring of patient progress and adherence to care plans, guiding PCC’s in making necessary adjustments to improve patient outcomes.
- Ensure PCCs are efficiently coordinating referrals to specialist, behavioral health services, and community resources, addressing care gaps or issues as they arise.
- Assist in identifying areas for improvement in care coordination process and work with the team to implement solutions that enhance patient outcomes.
Training and Development
- Lead training efforts for PCCs in key areas such as customer service, referrals communication, transportation coordination, behavioral health, and insurance navigation.
- Stay up to date with advancements in healthcare coordination and share insights with the team to continuously improve care practices.
Resource and Data Management
- Guide the team in connecting patients with appropriate insurance options, financial assistance, and community resources to support their care.
- Ensure that PCCs provide patients and families with educational materials and resources, empowering them to make informed healthcare decisions.
- Maintain oversight of accurate and up-to-date patient records and documentation across the team, ensuring compliance with healthcare regulations.
SKILLS & ABILITIES
Education: High School Diploma or General Education Degree (GED)
Experience:
- 2 years’ experience in patient care coordination, healthcare, customer service, or a related
role required. - Previous experience managing or leading small teams, ideally in a healthcare setting preferred.
- Strong understanding of healthcare systems, care coordination, insurance processes, and
community resources. - Excellent communication, interpersonal, and leadership skills with ability to foster a
collaborative team environment.
Computer Skills: Proficient in using electronic health records (EHR) and other healthcare software.
