<strong>Job Description:<br></strong><br>
</div><div>We are looking for a dedicated Home Care Physical Therapist to join our compassionate team in providing high-quality care to individuals in the comfort of their homes. As a Home Care Physical Therapist, you will play a crucial role in helping clients regain mobility, manage pain, and improve their overall well-being.<br><br>
</div><div>
<strong>Key Responsibilities:<br></strong><br>
</div><div>1. Conduct comprehensive assessments of clients' physical conditions in a home care setting.<br><br>
</div><div>2. Develop personalized and goal-oriented physical therapy plans based on assessment findings.<br><br>
</div><div>3. Administer therapeutic exercises, manual therapy, and other interventions to enhance clients' functional abilities.<br><br>
</div><div>4. Collaborate with clients, their families, and other healthcare professionals to ensure coordinated care.<br><br>
</div><div>5. Educate clients on proper techniques for home exercises and self-care.<br><br>
</div><div>6. Maintain accurate and thorough documentation of assessments, interventions, and progress.<br><br>
</div><div>
<strong>Qualifications:<br></strong><br>
</div><div>1. Degree in Physical Therapy from an accredited program.<br><br>
</div><div>2. State licensure as a Physical Therapist.<br><br>
</div><div>3. Strong clinical and interpersonal skills.<br><br>
</div><div>4. Ability to adapt therapy plans to meet the unique needs of home care clients.<br><br>
</div><div>5. Compassionate and patient-centered approach to care.<br><br>
</div><div>
<strong>Experience:<br></strong><br>
</div><div>Previous experience in home care or a related field is preferred. New graduates with a passion for home care are encouraged to apply.<br><br>
