About the role
Alliance Community South Australia is seeking an experienced NDIS Support Coordinator to join our established and experienced team based in Adelaide.
Our office is located in Unley, we offer a unique culture that provides you with the resources and support of a large organisation, whilst enabling you to focus on the day-to-day activities of your role.
This is a crucial position, which will be well suited to an experienced Support Coordinator with demonstrated experience in supporting complex participants. In this role you will be responsible for connecting participants with a National Disability Insurance Scheme (NDIS) Support Plan to individualised supports aligned to their NDIS plan.
Key functions/duties include, but not limited to:
- Support individuals with the aim of greater choice, control and independence in managing their personal lifestyle choices.
- Work closely with external and internal stakeholders to achieve positive outcomes for participants and the organisation.
- Provide varying levels of coordination across multiple stakeholders/providers
- Match participants with community based and/or mainstream programs to fulfil individual needs.
- Contribute to the development of service delivery through regular staff meetings, planning processes and policy reviews.
- Travel – You will be expected to travel and meet with Participants on a regular basis across Brisbane and occasionally the Sunshine Coast.
About you
Other key requirements of the role:
- Demonstrated knowledge of the NDIS
- Excellent communication skills – dealing with clients, internal teams (Support Workers etc.) and external stakeholders.
- Empathy and Patience – you will be dealing the more vulnerable people in our community.
- Relationship Building – you need to have strong influencing skills and the ability to build and develop key relationships.
- High level of problem solving skills, to successfully identify problems, develop solutions, and implement these using a logical and systematic approach
- Solid Computer Skills – Microsoft Office & Database Management
- Driving Licence – This role will involve a lot of driving to clients.
