Clinical Assessment & Admissions Nurse – Assisted Living

Position Summary
The Clinical Assessment & Admissions Nurse plays a vital role in ensuring residents are safely and appropriately admitted to our Assisted Living community. This position uses strong clinical judgment to complete pre-admission and ongoing resident assessments, establish appropriate levels of care, and initiate service plans in compliance with state regulations and community policies.
The Assessment Nurse collaborates closely with the Director of Nursing, Administrator, Sales and Marketing, and interdisciplinary teams to support smooth admissions, accurate care planning, and exceptional resident outcomes.
Key Responsibilities
Admissions & Resident Assessments

Review Discovery forms and supporting documentation submitted by the Sales and Marketing team

Schedule and complete pre-admission and admission assessments in a timely manner

Present assessment findings and recommendations to the Director of Nursing for admission determination

Review all admission denial decisions with the Administrator prior to final processing

Track resident status and reassess care levels as conditions change
Required Clinical Assessments & Care Planning
General Service Evaluation (GSE)
-
Complete and maintain a comprehensive evaluation packet upon admission, every six months, and as needed

Assess and document:
-
Wellness baseline
-
Pain levels
-
Mobility and transfer needs
-
Self-medication ability
-
Elopement risk
-
Cognitive status (mini-mental evaluation)
-
Level of Care determination
-
Communicate all Level of Care changes to the Assisted Living Coordinator and Business Office Manager
General Service Plan (GSP)
-
Complete GSPs using the electronic system (Visual)
-
Print, obtain signatures, and file updated plans in both the GSP binder and resident chart
Health Care Plan (HCP)
-
Initiate HCPs for residents receiving third-party services
-
Communicate all clinical findings and ADL needs to the DON and Assisted Living Coordinator

Complete reassessments at least every six months or with any significant change in condition

Notify residents, families, and/or POAs of any changes in care level
-
Notify the billing department promptly to ensure accurate charges
Documentation & Compliance

Maintain organized, accurate, and up-to-date documentation in designated GSE, GSP, and HCP binders
-
Bring required documentation to weekly Level of Care (LOC) meetings
-
Ensure all assessments are reviewed and signed by the RN

Document all communications with residents, families, POAs, physicians, and interdisciplinary team members

Ensure compliance with HIPAA, state regulations, and community policies at all times

Qualifications
-
Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing
-
Prior experience in assisted living, senior living, or clinical assessments strongly preferred
-
Strong communication, organizational, and documentation skills
-
Ability to work independently and collaboratively within an interdisciplinary team
-
Knowledge of HIPAA standards and regulatory compliance
