</div><div>Job Summary<br><br>
</div><div>Aachele Home Care and Hospice is seeking a compassionate and experienced Nurse Practitioner (NP) to provide high-quality palliative care services to patients in the Cobb County area. The Nurse Practitioner will work collaboratively with physicians, nurses, social workers, and interdisciplinary team members to deliver patient-centered care focused on symptom management, quality of life, and support for patients with serious or chronic illnesses.<br><br>
</div><div>Responsibilities<br><br>
</div><ul>
<li> Conduct comprehensive patient assessments in homes, facilities, or designated care settings </li>
<li> Develop and implement individualized palliative care treatment plans </li>
<li> Manage pain and symptoms related to chronic and serious illnesses </li>
<li> Order, perform, and interpret diagnostic tests as appropriate </li>
<li> Prescribe medications and treatments according to state regulations and clinical protocols </li>
<li> Educate patients and families regarding diagnoses, treatment options, and disease progression </li>
<li> Collaborate with physicians, hospice teams, case managers, and community providers </li>
<li> Maintain accurate and timely documentation in electronic medical records </li>
<li> Participate in interdisciplinary team meetings and care coordination </li>
<li> Provide emotional support and advocacy for patients and caregivers </li>
<li> Ensure compliance with state, federal, and company regulations and standards </li>
</ul><div>Qualifications<br><br>
</div><ul>
<li> Active Georgia Nurse Practitioner license </li>
<li> Master’s or Doctorate degree in Nursing from an accredited program </li>
<li> Board Certification as a Nurse Practitioner (FNP, AGNP, or related specialty) </li>
<li> DEA license and prescriptive authority in Georgia </li>
<li> Minimum of 1–2 years of experience in palliative care, hospice, geriatrics, oncology, or home health preferred </li>
<li> Strong clinical assessment and communication skills </li>
<li> Ability to work independently and manage a flexible schedule </li>
<li> Current CPR certification </li>
<li> Valid driver’s license and reliable transportation </li>

<li> Experience with home-based care </li>
<li> Knowledge of Medicare and Medicaid regulations </li>
<li> EMR documentation experience </li>
<li> Strong interpersonal and family communication skills </li>
